The review of Code of Practice- Rail Safety Investigations is now open for Public Consultation.
How will this help the rail industry?
This Code of Practice Rail Safety Investigations acknowledges a range of investigation models or frameworks that could be adopted to investigate rail safety occurrences. The Code provides the basis of a common approach to investigation of occurrences for the Australian rail industry that accommodates individually tailored, systemic investigation methods that RTOs may use.

RISSB is committed to providing a product that defines good practice for anyone that may be involved in the investigation of rail safety occurrences. The Code of Practice Rail Safety Investigation is based on the principle that rail safety investigations should determine the circumstances of occurrences, and any actions or recommendations identified should be developed to reduce risks from such occurrences.
The Code of Practice – Rail Safety Investigations has been reviewed and updated by a Development Group consisting of industry specialists.
As part of this review, the Development Group examined and addressed:
- An operator’s requirement to establish a set of well defined principles for investigations
- The adoption of a consistent approach for the analysis and review of investigation data
- The introduction of additional analytical tools to support investigations.
The Development Group recognised that a just culture approach to a rail safety investigation promotes transparency and encourages a positive safety culture. A just culture approach provides operators with valuable learning opportunities and ongoing continuous improvement.
How to contribute
The Code of Practice – Rail Safety Investigations will be available for public consultation until Friday 17 January 2025. All interested parties are encouraged to provide comments during this time.
